			<description>&#60;p&#62;I am trying to create a table or matrix on a form for contractors to access a building.  I want the titles to be Date Needed/Time/Freight Y/N/Loading Dock Y/N.  They need to have at least 4 lines where they can enter different dates/times, ect.  An example of this form in PDF is here:&#60;br /&#62;
