			<description>&#60;p&#62;Built a complex form, tested, and everything worked.  Today, the form shows up as an entry in WP Admin but when I preview it or try to edit it or view it in a page on the front end, none of the forms settings / fields show up ( &#34;Oops, form not found&#34; on front end, blank form on back end ).  Checked the database, the form has all of the meta data still intact.&#60;/p&#62;
&#60;p&#62;Any known causes for this sort of behavior ( to narrow our debugging )?
