Strangely, the "http://" prompt in the website field was causing the error. Once we changed that field to a regular text box field, everything worked normally. I did not see a confirmation message though. Sounds like that is the issue you're having? Post-migration, it has stopped accepting entries. When we click the submit button the form clears instead of directing to the Thank You page -- and there is no evidence of an entry being made on the form's backend.
