For some reason though it is only triggered when using W3TC (with minification). We don't really need the conditional logic of the form any more. Please put the conditional logic back on that form so we can take a look at it and see what the error is and can help you fix it.</p> <p>Removing the conditional logic just works around the problem. First I tried inserting a form which works on other pages and the page loaded successfully. Then I check what was different in the form used in <a href="http://www.2x.com/quote/" rel="nofollow">http://www.2x.com/quote/</a> from other forms and it was the conditional logic. I removed the conditional logic and everything worked great. We have W3TC installed but and this Javascript error is only generated when W3TC is enabled BUT other forms such as that in <a href="http://www.2x.com/applicationserver/download/" rel="nofollow">http://www.2x.com/applicationserver/download/</a> work without any problems even though cache is enabled on that page as well. Trying to debug but cannot come up with anything.<br /> Thanks </p>
