See this screenshot:</p> <p><a href="http://grab.by/9UDu" rel="nofollow">http://grab.by/9UDu</a></p> <p>The Sharebar script has an error which causes Javascript to quit executing so the form isn't displayed.</p> <p>Forms that have conditional logic use Javascript to show/hide only the fields that should be visible.
