			<description>&#60;p&#62;This isn't a permalink issue.  It's a Microsoft IIS URL rewriting issue.  It doesn't properly handle the permalinks.  Because the default permalinks use no url rewriting at all, that is why it works.  If you use permalinks that use URL rewriting, it's not going to work because of how your web host has implemented rewriting.  Older versions of IIS implement it as a hack, and that causes problems for WordPress and plugins.

I definitely wish my company had a non-windows server for all of our WordPress work, but unfortunately it's not something I have any control over.

&#60;p&#62;Can you submit the form from the preview (which by default will uses non-pretty permalinks.)&#60;/p&#62;
&#60;p&#62;And can you try without pretty permalinks active and see if it will submit?&#60;/p&#62;
&#60;p&#62;I submitted a test using this URL:&#60;br /&#62;
