Form doesn't display on page, either. I don't see any js errors. This is a fresh wp install (3.5.1), with no other plugins enabled. </p> <p>the form is quite long, and when I check the db directly, the entry_meta field is populated with the form data. It seems like it got cut off after the last entry though, so I'm wondering if the form data is too long?</p> <p>I think I may have messed up the db field, too - can you tell me what the very end of the entry shoudl look like?
