Cause for me was Google Maps for WordPress plugin. If you're having trouble with toolbar addons (like the gravity forms button) the best place to start looking is at any other plugins that add a toolbar button themselves. Typically with the "Insert Form" wizard it's a cache issue, and I still think it could be a cache issue with your site.</p> <p>The "Insert Form" wizard isn't the only way to insert a form, all it does is build the shortcode for you. You can just as easily insert the form by manually typing in the shortcode. The javascript has been cached by your browser so it's not loading the new javascript for this functionality to work properly.
