The form does not appear. </p> <p>I also placed the same form in a post using shortcode. The form does not appear but the following error does;</p> <p>'Oops! We could not locate your form.'</p> <p>I am assuming it is a PHP error. Does something else need to be activiated?</p> <p>Thanks! </p>
