I see the JavaScript is minimized in the head, but you have a bunch of JavaScript errors on the page. I think that's what's causing your troubles. I finally got the form to display by duplicating the original form, renaming it and then adding the "new" form to the page. Although, it didn't work to solve the issue on the page listed above. I'm embedding all forms via short codes. I'm using WordPress 3.2 and tried Safari, Firefox and Chrome.</p> <p>View source shows that the an inline style display:none is being added to the form. Not sure why. </p>
