What version of Gravity Forms are you running?

Something (most likely a plugin as Thesis doesn't do this as far as I am aware) is manipulating the shortcode output and adding paragraph breaks where they shouldn't be in the Javascript output. This is causing a Javascript error on your page which is why the form is still hidden as conditional logic uses Javascript to show/hide the parts of the form that should be visible.

See this thread:

http://forum.gravityhelp.com/topic/form-not-showing-at-allchecked-footernow-what

Note that the functionality in that thread (the noformat shortcode) sometimes uses other names such as the raw shortcode. We don't recommend adding this type of functionality to your site as it literally breaks how WordPress handles shortcode output which causes problems with plugins.

We did add a fix in a recent version of Gravity Forms that makes it play nicely with the noformat and raw shortcode functionality, which is why I asked which version you were running.

I have gravity forms working fine on about 10 other sites, all running Thesis, but it is giving me problems this time around and I have no idea why.

Checking out the source and CSS, gform_wrapper is getting "display: none" applied. I tried the footer adjustment suggested in the FAQ, but it only broke the site.

One of the forms is here, it does not display at all but I can see all of it in the source output.
