I tried that already and did it again but no joy. What I ended up having to do is install the plug-in on another WordPress site, then export / import the appropriate tables into the original site. I would suggest uninstalling the plugin completely, re-uploading and then re-activate the plugin. The plug-in says that the form is saved... but when I go tot he form list there is nothing there. </p>
