It's possible it is something at your location. But it is bizarre if so. There is no reason why it shouldn't work.</p> <p>Both those forms use conditional logic which relies on Javascript to show/hide elements of the form. If you have a firewall that is blocking the Javascript request for some reason, that would cause the form not to display.
