If you want this just on the category archive page, a good starting point would be the category.php file.

To have an embedded form at the top of a specific blog category, sort of like a Q&#38;amp;A.  Users would fill out form and an email send to an admin.  The Admin would then answer the question, the question and answer would then become a blog post listed chronologically.
