That one error that is showing was coming from a prettyPhoto function call. It looked like it doesn't have a parameter set. Follow the instructions above on checking for theme/plugin conflicts and let me know what you find out.</p> <p>If you want, you can also export your form and send the XML file to me to <a href="mailto:rob@rocketgenius.com">rob@rocketgenius.com</a> so I can take a look at the form itself while you test. Does that form use conditional logic? Does it show in the formbuilder preview area? It sounds like you might have a JS conflict of some kind. If I duplicate that form, rename it and embed the duplicate form into another page, it does not appear. The form is active and I am able to select it from the form list drop down and embed the form in the page, but when I go to preview the page the form does not appear. I did a test by creating a new page and new form and everything works<br /> What has happened to our original form that it does not embed properly anymore?</p> <p>Thanks for your help! </p>
