I had a couple thoughts though. The HTML in the top menu is invalid. Can you hide that menu, or fix the HTML, and see if that has an effect?</p> <p>Also, looks like you are using uniform.js. That has a direct effect on all your form elements. Can you disable that and check again? Does the form work correctly in the form preview? And will it work correctly if you try the Twenty Eleven theme from WordPress?
