			<description>&#60;p&#62;You should contact your theme provider for their support.    I had a couple thoughts though.  The HTML in the top menu is invalid.  Can you hide that menu, or fix the HTML, and see if that has an effect?&#60;/p&#62;
&#60;p&#62;Also, looks like you are using uniform.js.  That has a direct effect on all your form elements.  Can you disable that and check again?  For some reason, the next button and all the drop downs on the bottom have &#34;display:none&#34; added to them, in addition to the class for the 'uniform' script.

And will it work correctly if you try the Twenty Eleven theme from WordPress?  It looks like there is some sort of plugin or theme conflict causing the form not to work properly.
