I am trying to have a three column form where there is a box for quantity, an item name (already in place - not entered by user) and then a text box for special instructions.</p> <p>Like this:<br /> Quantity Item Special instructions<br /> ________ Bananas ________________<br /> ________ Oranges ________________<br /> ...etc.</p> <p>I do NOT want to include prices.
