			<description>&#60;p&#62;This is really a customization request and not a support request. If it were something minor, I wouldn't mind taking a few minutes, but it takes a decent amount of time &#38;amp; effort to create a custom madlib style form. That's out of scope for a support request. The tutorial covered the basics, but if you're not comfortable with CSS, I would suggest hiring a developer to do it for you.&#60;/p&#62;
&#60;p&#62;Here are a few good developers with Gravity Forms experience. I'm sure others could help you out as well.&#60;/p&#62;
