It looks like jQuery is not included so that is why things aren't working correctly. If you are embedding a form using a function call, you need to also enqueue the necessary Gravity Form scripts. Unfortunately, even with Ajax enabled within the shortcode... upon submit the entire page reloads and ideally I'd like the form to stay in focus but, display the validation errors.
