For the footer form, you should duplicate the "contact us" form so it has it's own form. This is because on the contact us page you have two instances of the same form, and they will not function correctly.</p> <p>When you duplicate the form and set the new one in the footer, make that form's label placement "top aligned" - then you'll just need to do a couple small CSS tweaks for padding/margins.
