The page just shows the short code on the screen and not the form itself. The theme I am using is based on the Twenty Eleven Theme. I know that it is something in the theme, because when I activate the Twenty Tweleve theme it shows up.</p> <p>Do I need to add something to my functions.php in order to get this to work? </p> <p>Please advise. </p>
