The error message above is the issue--can't post anything

For the dots, copy the following into you theme stylesheet:
.gform_wrapper ul li.gfield {
list-style: none !important;
}

As for your other issue, this only started when you installed a W3 plugin-in? Which one, and what exactly was it for?

My site is not live---I sent email with log-in info earlier--do I need to resend? (from admin@curriculumcafe.com)

I've posted forms but now I can't---getting error--
Request Entity Too Large
The requested resource
/wp-admin/post.php
does not allow request data with POST requests, or the amount of data provided in the request exceeds the capacity limit.

the only change was that I installed W3 plug-in to improve site speed as it was slow---so I deactivated it, cleared browser, cache, cookies--and still message

The discs are bullets related to your theme. Can you post a link to your form? You will have to remove them using CSS to override your theme putting them in.

I wanted the user_login and the date to show up auto on every form that was filled out. Am I wrong to assume that this is what the hidden field is for?
and if I'm using the hidden field properly---how do I go about removing the discs because everywhere I have a hidden field I have these discs with nothing showing that makes the form look tacky.
