That will result in a layout like I show here: <a href="http://minus.com/lbqvC9swpL4CDm" rel="nofollow">http://minus.com/lbqvC9swpL4CDm</a></p> <p>Then, add your fields before the "Page Break" section. If you want more page breaks, go ahead and click the "Page Break" button again. Just beware that each page needs to have form fields in it.
