You could do something similar using Ready Classes. You'd have to add a field for each Equipment Number, B&W and Color field. Then use the Ready Classes to position the fields side by side.

It won't be exactly like your example form, but it will be similar. Documentation for the CSS Ready Classes can be found here: http://www.gravityhelp.com/documentation/page/CSS_Ready_Classes

Thanks for your prompt response. Obviously, this will take some time to digest. I was curious if creating the "headers" was feasible as well? In the example link shown earlier, it would be the METER READS and EQUIPMENT headers above the fill-in fields. Does this involve more CSS coding?

Don't know if you are able to recreate these table borders from the original layout, but the rest is very easy. All you need are the CSS ready classes, described here: http://www.gravityhelp.com/documentation/page/CSS_Ready_Classes

My client has been using another form generator for which I used their html code. The form appears OK, but doesn't seem to be sending the data to them as desired. I don't see a way to replicate the complexity of the form with Gravity. Can you advise it is possible to create something like the one shown here?
