I see the fancy box working on the front end this morning but when I look at the form in the backend (still open from last night), there weren't any visible changes in the code like you mentioned. I cleared cache and apparently because the form was open, it must have saved the form as I had it on my browser. Your theme already has fancybox.js included, which is what your other product pages seem to be using. So I stuck with that. If you go into the Order Form and look at the 3rd item in the line, you'll see what I did.
