I found that the problem was with the theme (the "Awake" theme, from http://themes.mysitemyway.com/awake/). It contained a function that changed the sender name and email address to the administrator's name and email. Disabling the function fixed the problem. Thanks for the troubleshooting steps!

It's possible that a theme or plugin conflict can cause this.

http://www.gravityhelp.com/documentation/page/Testing_for_a_Theme/Plugin_Conflict

Gravity Forms uses the built in WordPress wp_mail() function to send email, the wp_mail() function has hooks of it's own and some plugins have been known to use these hooks to force the Send From address that the wp_mail() function uses. If you do that, it will override anything Gravity Forms passes to it.

I saw it ... but in the configuration for that plugin, it looks like you set a single "From" address for all outgoing messages. We have multiple forms on our website, and we don't necessarily want them to all have the same "From" address. Gravity Forms supposedly lets each form have its own, and that is a far more ideal solution. I was hoping to make it work.

Have you tried using the SMTP plugin yet? http://wordpress.org/extend/plugins/wp-mail-smtp/

I have a contact form set up at http://livetest.3abn.org/contact/

In the notification settings for this form, I have noreply@3abn.org set as the "From Email" for both Notification to Administrator and Notification to User. However, in testing, the "From Email" is replaced my personal email address in messages received by both the administrator and user. This is a big problem, because I do not want my personal email address attached to all notification messages. But I'm not sure where to look to fix it.

My web host is GoDaddy (Linux based), and I'm using WordPress 3.3.1.
