I have posted both forms on <a href="http://www.pepperbase.com/test" rel="nofollow">http://www.pepperbase.com/test</a>. The first one is made with GF. The second one is just an amazon s3 upload field I created with the help from the amazon developers site. Users shoud fill out the first form and than upload a movie file using the the second form.
