			<description>&#60;p&#62;I haven't tried it in the Preview…but I discovered that if I disabled the e-mail notifications that the error goes away. So, at least I've honed in the cause. I'm going to leave the e-mail notifications off for now…until I have some time to investigate further.

			<description>&#60;p&#62;I just added a form to my site at: &#60;a href=&#34;http://technicallysimple.com/survey-online-productivity-course/&#34; rel=&#34;nofollow&#34;&#62;http://technicallysimple.com/survey-online-productivity-course/&#60;/a&#62; – when I fill out the form and click submit I get an &#34;Internal Server Error&#34;. The form data does end up getting stored, but this error message comes up instead of the confirmation (I'm using &#34;text&#34; confirmation, but tried using &#34;redirect&#34; confirmation and got the same error).&#60;/p&#62;
&#60;p&#62;The other forms on my site, including the contact form (&#60;a href=&#34;http://technicallysimple.com/contact/&#34; rel=&#34;nofollow&#34;&#62;http://technicallysimple.com/contact/&#60;/a&#62;) work fine.&#60;/p&#62;
&#60;p&#62;Any idea what might be causing this error?
