You are putting the query string parameters in the wrong place. </p> <p>You put the URL in first text input box, and then you put the query string parameters with the form field variables in the textarea that appears underneath the "Pass Field Data Via Query String" checkbox. </p> <p>I made the change in your form settings, but i'm not sure how to test it out.
