Your fields aren't being displayed, which means the jQuery isn't executing properly. This could be due to a javascript error on the page. When the jquery is unloaded, the gform appears again.</p> <p>I know this has something to do with conditional logic, which is used in this gform. But that relies on jquery. How can loading jquery break it? </p> <p>Is there a fix for this? </p>
