Gravity Forms wasn't built to be a login form solution, so you would have to customize it using our hooks and PHP. You can easily redirect a user based on a selection using our new Confirmations system in Gravity Forms 1.7.

Hey guys --

I'd like to use Gravity Forms to create a form that collects which sub-domain the user is interested in logging into as well as their actual login and password, so that when they hit enter the form understands which site they're attempting to access and thus redirects them accordingly.

Success with this anyone?
