&#60;p&#62;I have searched and read into a lot of this, but sadly I was unable to find any information on placing a form without using a shortcode.&#60;/p&#62;
&#60;p&#62;Can anyone help me out here?  I want to place the form into a template without using a shortcode, just straight php.&#60;/p&#62;
