			<description>&#60;p&#62;takiyahnoble, when you add a form to a page template like this, you need to ensure the scripts and CSS are included.  It happens automatically when using the shortcode.  When you add the PHP to your template file, you need to make sure this happens on your own.  See here:&#60;/p&#62;
