&#60;p&#62;I think the issue occurs because you are including jQuery in the page twice.  The second time is in the footer, and is an older version (1.7.2):&#60;/p&#62;

&#60;p&#62;That is being added by the theme, which is why the input mask works when you change themes.  Remove that call to jQuery and ensure your theme still works and if there are no jQuery errors on the page, the input mask will work.
