You don't have to guess at these though. You should use the merge tag drop down. Is that not working for you? This solved the issue.</p> <p>I did find that the address form fields were sequential as expected, for example 14.1, 14.2, 14.3, etc. The name form fields were a bit different. First name was field 3.
