Check Formstack's product for ideas.</p> <p>Most notably, their Matrix field type comes in very handy for a First Choice, Second Choice, Third Choice scenario. Especially if you make so you can specify if rows, columns, or both should be unique. That is, each activity can only have one choice ranking and each choice ranking can only be applied to one activity.</p> <p>Thanks! </p>
