			<description>&#60;p&#62;This is not currently easy to do in Gravity Forms.  The functionality you're referring to is called a Likert Scale.   Related topics:&#60;br /&#62;

&#60;p&#62;You could use an HTML field to create your header row, then create individual rows for the things to be rated, and hide the labels for the individual radio buttons.  As I mentioned, it's not easy to do right now, but it is something we plan on adding in a future release.&#60;/p&#62;

			<description>&#60;p&#62;I have looked a few posts about multi-column radio buttons and have seen a few ways to change classes and things, but they don't see to address my issue.&#60;/p&#62;

&#60;p&#62;I can do everything except the 5th and 6th entry where I want to share a 5 column heading and have 6 different rows. But, I need each row to be individually select-able. Is someone able to help me (or point me to a previous discussion) about how I am able to complete this action?&#60;/p&#62;
