			<description>&#60;p&#62;Okay, I copied your markup and started removing things. I found that when I removed the &#60;a href=&#34;http://pastie.org/2933772&#34; rel=&#34;nofollow&#34;&#62;free ad stats code at the end of the page markup&#60;/a&#62;, the display issue went away so there's the source of the problem.&#60;/p&#62;

			<description>&#60;p&#62;The &#34;pages&#34; are all actually in the markup, but hidden until needed with an inline &#34;display:none&#34; style attribute. If you look at the markup you'll see the style declaration in place. If you inspect the markup with Firebug or another developer tool you'll find the style is being stripped. This is usually due to a theme/plugin conflict.. some script is modifying the markup by the time the browser renders it.&#60;/p&#62;
&#60;p&#62;If you've already switched themes and the issue persists, then I would turn to other plugins as the most probable source of the problem. You will just need to do some testing to find out exactly where the issue originates.&#60;/p&#62;

			<description>&#60;p&#62;Alex, have you tried activating one of the default WP themes, such as 2010 or 2011 and see if it works? Usually when the forms works in the Preview, but not on the site - it is a conflict of some kind.
