I don't see any JavaScript or jQuery errors, but I do see that you are loading jQuery from your theme folder, rather than enqueueing the WordPress default jQuery. I wonder if that is related? If possible, allow WordPress and Gravity Forms to enqueue their own jQuery and see if the form shows.</p> <p>Also, did you add the form to the page in any way other than the shortcode in the post editor?
