In both cases the fields do not show - only the title.</p> <p>Additionally, the fields DO work if you use the Preview Form link after creating the form, but not within the template - cleanr template. Clearly it must be related to the template, but I am not sure what is the root cause.</p> <p>Any thoughts? </p>
