When you go to the Mailchimp settings pages under the forms menu ( <a href="http://example.com/wp-admin/admin.php?page=gf_mailchimp&amp;view=edit" rel="nofollow">http://example.com/wp-admin/admin.php?page=gf_mailchimp&amp;view=edit</a> ), you first select your list (which is pulled dynamically from Mailchimp) and then select your form.
