&#60;p&#62;If you have individual pages, you can use the same form and embed it with the shortcode, but populate the team member name in the shortcode:&#60;/p&#62;

			<description>&#60;p&#62;I have a site that has multiple team members (25 or so). I was wondering if there was a way I could create one &#34;Team Member Contact&#34; form, then embed it into the team-member template file using custom fields gleaned from each team member. Basically I just need to duplicate a form over multiple pages but insert different custom field data (email_address &#38;amp; member_name) according to which team member's page you are on.&#60;/p&#62;
