I was wondering if there was a way I could create one "Team Member Contact" form, then embed it into the team-member template file using custom fields gleaned from each team member. Basically I just need to duplicate a form over multiple pages but insert different custom field data (email_address &amp; member_name) according to which team member's page you are on.</p> <p>Does this make any sense? </p>
