			<description>&#60;p&#62;This is a theme-related issue and not a Gravity Form issue. When I disable the main theme stylesheet, you'll see that the fields appear just fine in Safari.&#60;/p&#62;

&#60;p&#62;when i view this form in safari on a couple of different computers, the first input field (name first last) is missing. Looks fine in Firefox.
