I can do the customized layout just fine but I'm not sure what do to about the input fields I need to put in the HTML. Can I just add any fields I want and they'll get passed correctly or is there something unique I have to do for Gravity Forms?

&#60;p&#62;Honestly, aside from using strategically placed HTML fields for certain &#34;fake labels&#34; in conjunction with heavily styled/positioned inputs, I'm not sure.&#60;/p&#62;
&#60;p&#62;This would be a heavy customization job - even for me and I know Gravity Forms and CSS fairly well.

			<description>&#60;p&#62;This is why printed forms don't always translate to the web in terms of layout. Even if you were to get this laid out with some trickery using HTML fields and such, you wouldn't be able to line up the data or match up what is what in the entry.&#60;/p&#62;
&#60;p&#62;I'd suggest you try and do some convincing as to why this format wouldn't work on the web, as this layout lends itself to print.
