			<description>&#60;p&#62;I would like someone help me write some modifications, so when my form is submitted the fields are inserted into a separate database.  I have 3 different contact tables for different contact types.  Each Table list things like name, age, gender, nationality.  There are some sub-tables ie a list of nationalities which should be referenced by ID.&#60;/p&#62;
