I moved it back to the page I wanted it and it stopped working again. I figured out that it did not like being a sub-page of a parent page. I switched to using the menu option under appearance and created a customized menu there instead of using the wp page order. It views beautifully but when I do a test submission it: 1) does not register 2) if i miss a field that is required, it does not notify me 3) there is no way for the user to know the submission is not going through! What am I missing? I have deactivated the theme and used twenty ten and the same issues continue. I do not have very many plug ins being used and I have deactivated them and that doesn't fix it either.
