It's a mystery as to how this came to pass. None the less I had to reinstalled WP and now the plug in appears to be working fine. Thanks for your help. </p> <p>On a side note and just an FYI. The error occurred in media.php on line 377. This is a printf() output statement and it looks like the plug in some how wasn't talking to this file to add the new icon. I tested by turning the plug in off to see if it was in fact the plug in causing the problem. Sure enough when the plug in was inactive the problem disappeared.
