I lifted some code straight from <a href="http://codex.wordpress.org/TinyMCE" rel="nofollow">WordPress Codex</a>, put this is a plugin and encountered the exact same "symptoms" as the poster in <a href="http://www.gravityhelp.com/forums/topic/notifications-html-button-doesnt-work" rel="nofollow">this thread</a>. Deleting my TinyMCE customisations fixed the issue. </p>
