			<description>&#60;p&#62;You can get the form field variable (merge code) from using the &#34;Insert form field&#34; drop down on the Message or Subject fields.  You can then copy-n-paste it into any of the Notification fields.

			<description>&#60;p&#62;You can do this using custom fields.  Add a custom field for each page that stores the email address that you want to send the notification to.  You store it as a custom field value for that page.  You then populate a hidden field on your form with this custom field value, and then send the email notification to that value stored in that hidden field.&#60;/p&#62;

			<description>&#60;p&#62;Is there a way to have a single contact form to be embedded into multiple team member pages and automatically route that email to the member based on the page it is from.&#60;/p&#62;
&#60;p&#62;Example:&#60;br /&#62;
There are 3 pages: John, Sue, Bob&#60;br /&#62;
Each page has the same Gravity Form with standard contact questions.&#60;br /&#62;
The page is titled John, Sue, Bob&#60;br /&#62;
When someone submits a contact request, it automatically chooses the right email address based on the page it is from.&#60;/p&#62;
