Once I did that and saved it, it now shows up again.</p> <p>I didn't think I had added any conditional logic, but had it checked for one of the elements. Once I unchecked the conditional for that element the old one began working again as well. When I determined that it was not my theme or other plugins installed... I created a duplicate of the form that was causing the problem and inserted it into the page. I then started removing fields from the duplicate form, and concluded that it was the PayPal Total field causing the problem. Removed the existing one, and replaced it with a new PayPal Total, and the form displayed perfectly.
